摘要

目的探讨巴塞罗那标准在合并完全性左束支阻滞患者中诊断急性心肌梗死的效能。方法回顾性分析136例完全性左束支阻滞患者经过心肌损伤标记物和冠脉造影检查

确诊合并急性ST段抬高型心肌梗死患者56例作为观察组

未合并急性心肌梗死的患者80例做对照组。应用巴塞罗那标准判读心电图。结果巴塞罗那标准诊断完全性左束支阻滞合并急性心肌梗死的敏感性94.6%（84.2%-98.6%）

特异性88.8%（79.2%-94.4%）;阳性预测值85.5%（73.7%-92.7%）

阴性预测值95.9%（87.8%-98.9%）。结论巴塞罗那标准具有较高的敏感性和特异性

且简单易记

临床应用价值大。

Abstract

Objective To evaluate the diagnostic efficacy of BARCELONA algorithms in acute myocardial infarction complicated with complete left bundle branch block. Methods The electrocardiogram and the myocardial injury bio-markers and the coronary angiography of 136 patients with complete left bundle branch block were retrospectively analyzed. Fifty-six patients with acute ST-segment elevation myocardial infarction were selected as the observation group and 80 patients without acute myocardial infarction were selected as the control group. Results The sensitivity and specificity of BARCELONA algorithms in the diagnosis of acute ST-segment elevation myocardial infarction in the presence of left bundle branch block were 94.6%（84.2%-98.6%） and 88.8%（79.2%-94.4%）. The positive predictive value was 85.5%（73.7%-92.7%）

and the negative predictive value was 95.9%（87.8%-98.9%）.Conclusions BARCELONA algorithms has high sensitivity and specificity in the diagnosis of acute ST-segment elevation myocardial infarction in the presence of left bundle branch block.
