目的 通过构建急性心肌梗死(AMI)动物模型,观察急性心肌缺血过程中J-T融合波的发生情况;于急性ST段抬高型心肌梗死(STEMI)患者,探讨急性缺血性J-T融合波与恶性室性心律失常(VA)之间的关系。方法 (1)对6只贵州小型猪进行静脉麻醉后,应用球囊封堵前降支(LAD)近段以制备AMI动物模型,记录并分析冠状动脉闭塞90分钟内急性缺血性J-T融合波的发生情况;(2)连续收集2018年4月至2021年1月于我院确诊且资料完整的338例STEMI患者的临床资料,根据心电图表现将其分为急性心肌缺血性J-T融合波组和急性心肌缺血性J-T非融合波组,明确急性心肌缺血性J-T融合波是否为STEMI患者发生院内恶性VA的独立预测因素。结果 (1)全部6只猪均完成LAD封堵,2只分别在缺血后73和13分钟出现了急性心肌缺血性J-T融合波,其中1只发生了室颤;(2)在338例STEMI患者中,95例(28.11%)记录到急性心肌缺血性J波,其中13例(13.83%)为J-T融合波,82例(24.26%)为J-T非融合波。表现为急性心肌缺血性J-T融合波的患者梗死相关血管(IRA)多为LAD(P=0.003),且左室射血分数(LVEF)更低(P=0.006)。多因素Logistic回归分析结果表明,急性心肌缺血性J-T融合波(OR 10.787
P=0.004)和Killip分级≥3级(OR:4.720
P=0.034)是STEMI患者住院期间发生恶性VA的独立预测因素。结论 构建AMI动物模型可诱导产生急性心肌缺血性J-T融合波;在STEMI患者中,约3.83%表现为急性心肌缺血性J-T融合波,其是STEMI患者住院期间发生VA的一个独立预测因子。
