摘要

目的探讨不同类型心力衰竭患者QTc间期、QRS-T夹角及心率变异性（HRV）与心功能的相关性。方法选取心衰患者103例，男57例，女46例，根据左心室射血分数（LVEF）分为射血分数保留的心衰（HFpEF）组43例、射血分数中间值的心衰（HFmr EF）组23例、射血分数降低的心力衰竭（HFr EF）组37例；另选取同期非心衰患者52例。完善心电图、动态心电图等相关检查。结果心电图QTc间期及QRS-T夹角在HFmr EF组及HFr EF组显著大于对照组及HFpEF组（P<0.01）；心率变异性时域指标中的SDNN、SDNNindex及三角指数在HFmr EF组及HFr EF组显著低于对照组及HFpEF组（P<0.01）；但HFpEF组与对照组相比各指标差异无统计学意义（P>0.05）；相关性分析显示，心衰患者LVEF与QTc间期及QRS-T夹角呈显著负相关性（r=-0.425

P<0.001)。结论心衰患者的QTc间期、QRS-T夹角及HRV与心功能存在相关性，可用于辅助判断心衰类型及指导治疗。
