目的探究P波离散度(Pd)及最大时限(Pmax)与阵发性心房颤动(PAF)及房性心律失常(AR)的相关性。方法选取于2018年1月

月在本院诊治的PAF患者30例及AR患者30例作为研究对象
将其Pd、Pmax与30例体检健康人员做对比观察。结果 PAF组患者Pd与Pmax值均明显大于对照组;AR组Pd与Pmax值均明显大于对照组;PAF组患者Pd、Pmax与AR组对比无明显差异。PAF组Pd≥40ms、Pd≥110ms、Pd≥40ms+Pmax≥110ms比例明显高于对照组;AR组Pd≥40ms、Pd≥110ms、Pd≥40ms+Pmax≥110ms比例明显高于对照组;PAF组与AR组两组对比无明显差异。Pd≥40ms+Pmax≥110ms条件下检测PAF与AR特异性最高
且阳性预测值最高
但与Pd≥40ms及Pd≥110ms之间的差异并不显著。结论经研究结果证实
Pd与Pmax可作为预测PAF、AR的生理指标
为临床诊断提供可靠依据
而将Pd与Pmax进行联合应用
能够进一步提升PAF、AR的阳性预测值。
Objective To investigate the correlation between P wave dispersion(Pd) and maximum time limit(Pmax) and paroxysmal atrial fibrillation(PAF) and atrial arrhythmia(AR). Methods 30 patients with PAF and 30 patients with AR who were all treated in our hospital in one stage were selected as subjects
and their Pd and Pmax were compared with those of 30 healthy people. Results the values of Pd and Pmax in group PAF were significantly higher than those in control group.; the values of Pd and Pmax in group AR were significantly higher than those in control group. There was no significant difference in Pd and Pmax between group PAF and group AR. The proportion of Pd ≥40 ms

Pd ≥40 ms+Pmax≥110 ms in the PAF group was significantly higher than that in the control group; The proportion of Pd≥40 ms

Pd ≥40 ms+Pmax≥110 ms in AR group was significantly higher than that in control group; and there was no significant difference between the PAF group and the AR group.The highest specificity of PAF and AR were detected under Pd ≥40 ms+Pmax≥110 ms and the positive predictive value was the highest
but the difference between Pd≥40 ms and Pd≥110 ms was not significant. Conclusion Pd and Pmax can be used as physiological indexes to predict PAF and AR
and provide reliable basis for clinical diagnosis.The combination application of Pd and Pmax can further improve the positive predictive value of PAF and AR.
