摘要

目的

分析QRS时限联合系统免疫炎症指数（Systemic Immune-inflammatory Index，SII）对急性心肌梗死（Acute Myocardial Infarction，AMI）患者接受经皮冠状动脉介入治疗（Percutaneous Coronary intervention，PCI）预后的预测价值。

方法

选取2021年1月

至2024年1月于连江县总医院行PCI治疗的85例急性心肌梗死患者临床资料，进行回顾性分析。根据预后情况将患者分为预后不良组（

=18）和预后良好组（

=67），通过医院电子病历系统收集患者的一般资料，并计算QRS时限、SII指数。采用受试者工作曲线（ROC）分析QRS时限联合SII指数对患者预后的预测价值。

结果

两组患者高血压史、糖尿病史、心肌肌钙蛋白I、QRS时限、SII指数等资料比较，差异有统计学意义（

0.05）；二元Logistic回归分析显示，影响急性心肌梗死患者介入治疗预后的影响因素有糖尿病史、心肌肌钙蛋白I、QRS时限、SII指数（

0.05）；ROC曲线分析结果显示，QRS时限、SII指数及两者联合预测急性心肌梗死患者介入治疗预后的AUC分别为0.745（95%

0.596~0.895）；0.822（95%

0.733~0.911）；0.984（95%

0.956~1.000）。

结论

QRS时限和SII指数都是能够反映患者预后情况的重要指标，两者联合使用在急性心肌梗死患者介入治疗的预后监测中具有重要的临床价值，有助于进一步改善患者预后。

Abstract

Objective To analyze the predictive value of the QRS duration combined with the Systemic Immune-Inflammatory Index (SII) for the prognosis of patients with acute myocardial infarction receiving percutaneous coronary intervention (PCI). Methods A retrospective analysis was conducted on the clinical data of 85 patients with acute myocardial infarction who underwent PCI in our hospital from January 2021 to January 2024. According to their prognosis

patients were divided into a poor prognosis group (

=18) and a good prognosis group (

=67). General information was collected through the hospital's electronic medical records

and the QRS duration and SII were calculated. The predictive value of the QRS duration combined with the SII for patient prognosis was analyzed using Receiver Operating Characteristic (ROC) curves. Results Comparisons of the history of hypertension

QRS duration

and SII index between the two groups showed statistically significant differences (

0.05). Binary Logistic regression analysis indicated that the influencing factors affecting the prognosis of acute myocardial infarction patients undergoing intervention treatment included the history of diabetes

0.05). The results of the ROC curve analysis showed that the AUCs for predicting the prognosis of acute myocardial infarction patients undergoing intervention treatment using QRS duration

and their combination were 0.745 (95%

and 0.984 (95%

0.956~1.000)

respectively. Conclusions Both QRS duration and SII index are important indicators that reflect the prognosis of patients. The combined use of these two indicators has significant clinical value in monitoring the prognosis of acute myocardial infarction patients undergoing intervention treatment and may help further improve patient outcomes.
